Mable's son, 14 year old Jimmy, likes to dress in his sister's clothes. According to the DSM IV on cross-gender identity, what are some of the tell-tale signs that Jimmy is devoping a serious disorder?
ANSWER: Myth, Stereotype, and Cross-Gender Identity [cross-gender DSM IV]
    A. A strong and persistent cross-gender identification (not merely a desire for any perceived cultural advantages of being the other sex). In adolescents and adults, the disturbance is manifested by symptoms such as a stated desire to be the other sex, frequent passing as the other sex, desire to live or be treated as the other sex, or the conviction that he or she has the typical feelings and reactions of the other sex.
    B. Persistent discomfort with his or her sex or sense of inappropriateness in the gender role of that sex. In adolescents and adults, the disturbance is manifested by symptoms such as preoccupation with getting rid of primary and secondary sex characteristics (e.g., request for hormones, surgery, or other procedures to physically alter sexual characteristics to simulate the other sex) or belief that he or she was born the wrong sex.
[It's not looking good for Jimmy!]

Mabel's Uncle Billy Bob has an eleven year old son who has recently developed Tourette's Disorder. The family would like to get him into a free study and treatment. See if the National Institute of Mental Health (NIHM) has a clinical study which Billy Bob, Jr. can get into.
ANSWER: National Institute of Mental Health does have such a study. Children with a recent onset of Obsessive-Compulsive Disorder or Sydenham's Chorea or Tourette's Disorder (particularly if associated with dramatic, severe exacerbations) are being sought for a treatment study comparing immunomodulatory therapies, such as plasmapheresis, intravenous immunoglobulin and prednisone. Children with episodic OCD or Tourette's Disorder are also needed for participation in a trial of penicillin prophylaxis against infection-associated symptom exacerbation. Looks promising. Have Billy Bob call Ms. Lougee, M.S.W., Child Psychiatry Branch, 301-496-5323.
Jimmy's brother, Willie, steals from all of the local stores. Mable has had to go to K-Mart and Target to get Willie after he has been caught taking toys. Dannon Kindler, an expert on kleptomania, has written an article on it for Clinical Pharmacology. Name the disorders which Dannon Kindler suggests have a comorbidity to kleptomania. PubMed, [http://www.ncbi.nlm.nih.gov/htbin-post/Entrez/query?db=m] will allow you to search Medline for this article.
ANSWER: Kleptomania, one of the rare impulse-control disorders, is characterized by an irresistible impulse to steal objects not needed for personal use or monetary value. There is a comorbidity between mood disorders, eating disorders, anxiety disorders, personality disorders, and kleptomania. Several recent case reports have suggested that serotonin reuptake inhibitors could be effective in the treatment of obsessive-compulsive spectrum disorders and specifically in kleptomania.

Bubba, Mable's co-dependent husband, was brought up in a family of alcoholics where Bubba watched after his alcoholic parents and learned to cover up the alcohol use for fear of discovery. Is co-dependence a mental disorder? What are some of the symptoms Bubba might show?
ANSWER: Try Codependence: Painful Adult Behaviors Learned in Childhood by H. Dan Smith, EDD, MFCC for a very good description and bibliography of co-dependence and its causes. Bubba probably takes a great deal of Mable's time and energy since he lacks self-esteem, worries, is easily hurt, and lacks a strong value system of his own.
